Rhinoplasty: Case 1

This is an attractive patient who has a very angled and masculin face but he has a large hump, the tip is pointing down and his cheeks are flat. This is a typical plunging tip deformity, which means that when the patient is smiling, the tip points even further down and the upper lip pulls up. It make the hump seem even bigger. We chose an open approach to work on the nose. It is done through a small incision between the nostrils. The scar is inconspicuous after a few months. On after pictures while the patient is smiling, the tip is stable and does not plunge anymore. This is acheieved by working on the septal muscle and placing cartilage grafts. The base of the nostrils was also narrowed. Besides, notice how flat and retruded the cheek were before surgery. That makes the hump stick out even more. We chose fat grafting as the method of augmenting the cheeks naturally, at the time of rhinoplasty to create a better facial balance.
